It would be my main suspect for the cause. I use API root tabs. As long as they are pushed well into the substrate you shouldn't have any problems with them leaching into the water and the plants thrive on them.

I've also had questionable results with bacteria in a bottle products.
These days I just plant up the tank, let it run for around 6 weeks and then introduce the fish a few at a time, like in the old days. No messing around with ammonia and all that malarkey.

It's only something to worry about if you don't like the look of it, which I definitely do not. Badly affected plants do seem to suffer to some extent as well as looking horrible.
