Has your dad been over-feeding or has someone been throwing in bread ?
Just asking.

Over feeding is a possibility, he has some very big koi in there. Definitely no bread or anything. You think that's the problem?

This is dropsy. Caused by a fatal build up of fluid in the body cavity. Unlikely to be kidney or liver failure if several have it. More likely to be a bacterial infection... I’d treat with Myxazin P ASAP to save the koi (other bactericides are available...).
The fish is a goldfish.

Dad is old school (or ignorant) depending on your point of view and doesn't believe in chems or meds etc. He likes to let nature do its own thing.
Meanwhile I order myxazin on prime and will put it in cheers mate

Myxazin P. It’s Myxazin for ponds (cold water has some different pathogens).
